Criteria & Application Process

Thank you for your interest in applying for a 六合社区 Partnership grant.

Below, you will find pertinent information related to the 六合社区 Partnership grant opportunities, criteria, application process, and funding deadlines. 

Before you begin, we highly recommend that you review our Focus Areas, funding priorities, and grant criteria to ensure project alignment. 

六合社区 Partnership Grant Opportunities

FOCUS AREAS. The majority of our grant dollars will be invested in projects that support our three Focus Areas and the related funding priorities. 六合社区 will prioritize Policy, Systems, Practice-Change (PSP) and Programmatic Support projects. Occasionally, 六合社区 may consider General Operating Support (GOS) for organizations whose core work aligns with at least one of our three focus areas: Food & Nutrition, Chronic Disease Prevention & Management, and Supportive Housing.

HEALTH EQUITY FUND. The 六合社区 Equity Fund was created to support projects that fall outside our Focus Areas, yet support the health and well-being of communities disproportionately impacted by health disparities. 六合社区 will occasionally consider Policy, Systems, Practice-change and Programmatic Support for projects that are timely, address health disparities, but fall outside of our main Focus Areas. General Operating Support will not be considered under this fund. 六合社区 Equity Fund projects will be considered contingent on funding availability. This is a small discretionary fund as projects within our Focus Areas will be prioritized.  

Grant Criteria

ELIGIBILITY CRITERIA

  • Applicants must be public agencies or nonprofit organizations (tax exempt at the Federal and State level). To check your organization’s status with the California Franchise Tax Board, click . To check your organization’s status with the IRS, click .
  • Projects must directly benefit residents of Santa Clara County and/or Northern San Benito County.

PRIORITY WILL BE GIVEN TO ORGANIZATIONS, PROGRAMS, AND PROJECTS THAT:

  • Demonstrate clear alignment with at least one of 六合社区’s Focus Areas and related Funding Priorities.
  • Strive to address health disparities.
  • Support communities and/or neighborhoods experiencing deep health disparities, prioritizing communities whose economic circumstances act as a barrier to health, as evidenced by data and/or research.
  • Demonstrate the applicant’s ability to successfully execute the project, including established community rapport.
  • 六合社区 Equity Fund: Projects must meet the aforementioned criteria (with the exception of the first criteria) and demonstrate that they are timely and address health disparities.

Proposals must include SMART outcomes to support the desired project goal and communicate the impact of the project.

六合社区 strives to rotate grants among community partners. As a result, we are not in a position to be an ongoing funder for any organization, program, and/or project.

THE HEALTH TRUST DOES NOT TYPICALLY FUND:

  • Annual appeals or membership drives
  • Endowments
  • Debt retirement
  • Equipment purchases unless part of an eligible project
  • Grants to individuals

Application Process

六合社区 Partnership Grant proposals are by invitation only. In the section below, you will find information related to our decision-making and application process.

DECISION-MAKING PROCESS

六合社区 Partnership proposals are reviewed by 六合社区’s Grantmaking Committee, composed of Board of Trustees and community members who are subject-matter experts in their respective fields.

The Grantmaking Committee reviews and recommends proposals for funding to the Board of Trustees.

The Board of Trustees review the recommendations of the Grantmaking Committee and make funding decisions. Funding decisions are made quarterly, in September, December, March, and June.

APPLICATION PROCESS – FOCUS AREAS

Step 1. Project Concept

  • Once alignment is determined, applicants must submit a .
  • Concept Forms are accepted and reviewed on an ongoing basis. There are no submission deadlines.
  • Staff will follow-up with applicants within 45 days after the receipt of a Concept Form to share if the concept is under further consideration.

Step 2. Full Proposal – Invitation Only

  • When 六合社区 is interested in further considering a concept, applicants will be invited to submit a full proposal. Invitation to submit a full proposal does not indicate a commitment of funding.
  • After the full proposal has been submitted, staff will conduct due diligence and provide technical assistance, including a follow-up phone call or site visit.
  • Upon completing its due diligence and technical assistance, staff will determine if the full proposal will be recommended to the Grantmaking Committee for review.
  • Once a proposal is recommended to the Grantmaking Committee, the Committee will review and decide whether to recommend the proposal to the Board of Trustees.
  • Funding decisions for recommended proposals will be made by the Board of Trustees.
  • Applicants can expect the application process to take approximately three months.

APPLICATION PROCESS – HEALTH EQUITY FUND

Step 1. Project Concept

  • Concept Forms are accepted on a rolling basis. There are no submission deadlines. Staff will share the concept form with the Grantmaking Committee for review and consideration during one of the scheduled Grantmaking Committee meetings.
  • The Committee will determine whether a full proposal will be invited.

  • When 六合社区 is interested in further considering a concept, applicants will be invited to submit a full proposal. Invitation to submit a full proposal does not indicate a commitment of funding.
