San Jose, CA  鈥 On March 27, 2024, 六合社区鈥檚 Board of Trustees approved four new grants totaling $467,500. The grants are part of quarter three of 六合社区鈥檚 fiscal year 2023-2024 $2.3 million grantmaking budget, supporting community organizations and public agencies aligned with 六合社区鈥檚 mission of building health equity in Silicon Valley. 

Three 六合社区 Partnership Fund grants have been awarded: two in alignment with the Supportive Housing focus area to Bill Wilson Center and Mosaic America; one to Asian Americans for Community Involvement (AACI) aligned with the Chronic Disease focus area. Fostering Promise received a grant aligned with the 六合社区 Equity Fund.

Together, the approved grants aim to improve health in our community by integrating wellness and nutrition services into senior and youth transitional housing; strengthening nonprofit organizational capacity for advocacy; and improving access to HIV care for at-risk community members.

六合社区 Partnership Fund Grants

六合社区 Partnership Grants support organizations, projects, and programs that align with our mission and three focus areas: Food and Nutrition, Chronic Disease Prevention & Management, and Supportive Housing.

Asian Americans for Community InvolvementTo pilot a new referral process to streamline and optimize referrals to AACI鈥檚 health clinic that will eliminate barriers to accessing care for HIV prevention and medications like PrEP/PeP for marginalized and at-risk community members.

Bill Wilson
CenterTo pilot a Nutrition Coaching Program to equip formerly homeless youth and young adults with the knowledge and skills to make healthy nutrition choices for themselves and their families.

Mosaic
America
To establish the efficacy of culturally-rooted pathways to mitigate social isolation and improve health outcomes for low-income Asian seniors in affordable housing.

First 5 San Benito, Author at N&R SpotlightAs a funder, 六合社区 supported First 5 San Benito as they opened a community commercial kitchen, cold storage, and food pantry at their site in Hollister. The ribbon cutting ceremony celebrated the launch of a project years in the making: the facility will be the first publicly available certified commercial kitchen space in San Benito County. First 5 received 六合社区 Partnership funding as a part of 六合社区鈥檚 Food and Nutrition Focus Area, which prioritizes investing in food organizations through infrastructure and capacity-building support.

Food insecurity continues to be a pressing and growing issue across low-income and marginalized communities. While many residents were already experiencing hardships before the pandemic, the financial and health impacts have continued to worsen over time. Over 8% of people in San Benito County experience food insecurity, and food has remained the number one item requested by First 5 program participants.

My name is Makayla, my experience with 六合社区 Trust has been outstanding!

My case worker Terry B really went above and beyond to make sure all my needs were met for me and my 4 year old to be housed as soon as possible. I’m very grateful to have had her assigned to me because I felt like she genuinely cared and wasn’t just doing her job.

If it wasn’t for 六合社区 I would still be homeless. Even after moving into my new apartment Terry has checked in with me multiple times to make sure I was okay, which really meant a lot to me as well. I was homeless since I was 16 so having a safe place to call home and get ready for work is very important to me especially as a mother. I found a two bedroom apartment I fell in love with and Terry helped me get my deposit paid and made sure my landlord received it as fast as possible so I could move in right before the holidays. Our Case Managers are compassionate and knowledgeable. They understand every client is different and each individual requires a personalized set of supports to thrive as Makayla and her child are presently doing. National Black HIV/AIDS Awareness Day (NBHAAD) February 7, 2024

Unveiling the Power of Unity: NBHAAD 2024 Theme – “Engage, Educate, Empower: Uniting to End HIV/AIDS in Black Communities”

This year, the spotlight shines on the powerful theme, “Engage, Educate, Empower: Uniting to End HIV/AIDS in Black Communities” for National Black HIV/AIDS Awareness Day (NBHAAD). This observance is a pivotal moment to recognize the disproportionate impact of HIV on Black individuals.

While significant strides have been made in reducing HIV within Black communities, persistent challenges such as racism, discrimination, and mistrust in the healthcare system continue to influence the access and utilization of HIV prevention services. These barriers also contribute to a decreased likelihood of engaging in HIV treatment and care.

NBHAAD serves as a crucial opportunity to amplify efforts in HIV education, testing, community involvement, and treatment specifically tailored to address the unique challenges faced by Black communities. By fostering unity and understanding, we can break down these barriers and work together to end HIV/AIDS disparities in Black communities.

Love Positive Women 鈥 February 1 – February 14, 2024 Valentine’s Day

Love Positive Women 2024 is not just an event; it’s a global movement that transforms Valentine’s Day into a powerful platform for expressing love and care for women living with HIV.

Conceived by Visual AIDS artist member Jessica Whitbread in 2013, this ongoing project has become a beacon of solidarity.

As part of this initiative, participants craft heartfelt messages of support, turning them into Valentine’s cards that traverse the globe to reach HIV-positive women in time for Valentine’s Day. The significance goes beyond a simple gesture; it addresses the profound challenges faced by women living with HIV, such as isolation, stigma, and unique struggles in relationships, motherhood, and healthcare.

Since its inception in 2015, Love Positive Women has sent thousands of these cards worldwide, a testament to its commitment to eradicating stigma, fostering community, and empowering women living with HIV. It’s a celebration of love that serves as a powerful reminder that together, we can make a difference.

Take a Look at the Bounty; End of Year FEMA funds for the Jerry Larson Food Basket

六合社区鈥檚 HIV/AIDS Services department received a large amount of end-of-year funding from FEMA, MAC Cosmetics, and Broadway Cares Equity Fights AIDS. With these resources, the Jerry Larson Food Basket (JLFB) was able to purchase food pantry items for HIV/AIDS Services clients.听 This money was also used towards meeting the HIV/AIDS Service food quality improvement project, increasing the offerings of culturally appropriate and diet restricted foods.

We received a massive food delivery at the end of December. Special shout-out to Food Basket Manager, Barry Noll who came in over the holidays to receive the food and the HIV/AIDS Services Case Managers who unloaded four trucks of food in one day, while still attending to their busy client schedules. On January 4, 2024, eighteen Volunteers and corporate partners worked to sort and prepare the food for distribution to our community. It takes a village!

Thank you to all who helped make this robust amount of food accessible to those in our community who need it most!

The grants are part of quarter two of 六合社区鈥檚 fiscal year 2023-2024 $2.4 million grantmaking budget, supporting nonprofit organizations and public agencies aligned with 六合社区鈥檚 mission and focus areas.听

Three grants awarded out of the foundation鈥檚 六合社区 Partnership Fund mark year two of a three-year pilot program to support people with Type 2 diabetes through partnerships with community health centers: Gardner 六合社区 Services, Indian 六合社区 Center of Santa Clara Valley, and Roots Community 六合社区 Center, South Bay. Two additional 六合社区 Partnership grants were also awarded to First 5 San Benito and Rahima International Foundation.听

The City of San Jose Parks, Recreation, and Neighborhood Services received a grant as a part of the 六合社区 Equity Fund.听

Together, the projects will build health equity by improving health outcomes amongst individuals with Type 2 diabetes; strengthening the infrastructure of the food safety net in San Benito County; providing culturally preferred foods to families and individuals experiencing food insecurity; and supporting a cross-sector partnership to impact key areas contributing to health.听

In December 2020, 六合社区 awarded a grant to to conduct a literature review centering on African Ancestry maternal and infant health.听 This comprehensive review played a pivotal role in shaping the development and implementation of a culturally responsive pilot program, “Food as Medicine,” specifically targeting women of African Ancestry in Santa Clara County.

Recently, Roots Community 六合社区 Clinic and 六合社区 had the privilege to present at the in Kansas City. Alma Burrell, Chief Administrative Officer, and Nkemka Egbuho, Program Coordinator from Roots, and Shaunta McGee, our Vice President of Programs, led a fireside chat on “Food as Medicine: A Fresh Approach to Black Maternal 六合社区.”

Firstly, I had an absolutely amazing time at the conference!

One of the highlights for me was being given the opportunity to facilitate a fireside chat with Alma Burrell and Nkemka Egbuho, MPH. Our discussion centered around the Root Community 六合社区 Clinic’s innovative approach to Black Maternal 六合社区 intervention, specifically their “Food as Medicine” program for pregnant and postpartum black women in Santa Clara County. It was truly inspiring to hear about their efforts to address this critical issue.

In addition, I had the pleasure of meeting some incredible individuals at the conference, including Susan M Wolfe, PhD and Ann Price. Their passion for creating positive change within their respective communities was truly infectious. I’m excited about the potential collaborations and connections that may arise from these encounters.

Moving forward, I would love to stay in touch with these amazing women and continue our discussions on shared passions and initiatives. Let’s keep each other updated on our progress and explore ways we can support one another’s work.

Once again, thank you for providing such a meaningful platform through the Root Cause Coalition National Summit. It has been an enriching experience that has further fueled my commitment to making a difference.


The Root Cause Coalition鈥檚 Annual National Summit brings together representatives from health systems, hospital associations, foundations, businesses, national and community nonprofits, health insurers, academic institutions and policy centers to share best practices. To finally achieve health equity we must work together, across sectors of service, to address systemic racism and the social determinants of health.

Supervisor Chavez provided an overview of her proposed recommendations related to Reid-Hillview Airport and unleaded fuel, which will be discussed by the Board of Supervisors on 12/12/23

Michele Lew from 六合社区 spoke about the harmful effects of lead on children.

Local residents Ernesto and Maricela spoke about the challenges and problems with living near the airport.

In 2022, Santa Clara County banned the sale of leaded aviation gas at Reid-Hillview Airport. Local advocacy groups supported the efforts, recognizing the harmful impacts of lead poisoning.

Over the last few years, 六合社区 has participated in advocacy to close Reid-Hillview Airport and ban the sale of leaded gas at the airport, shining a light on the health impacts on local children who live, play, and go to school near the airport.听

六合社区鈥檚 CEO Michele Lew joined County Supervisor Cindy Chavez, former County Supervisor Blanca Alvarado, and neighborhood advocates at a press conference on Dec. 11th to continue advocacy against leaded fuel at community airports such as Reid-Hillview.听

The has declared that emissions from aircrafts that use leaded fuel pose a danger to public health. And according to the Centers for Disease Control, protecting children from exposure to lead is important to lifelong good health. No safe blood lead level in children has been identified. Even low levels of lead in blood have been shown to negatively affect a child鈥檚 intelligence, ability to pay attention, and academic achievement.听

Our county should continue to move forward in keeping leaded fuel away from children, and providing safe spaces for youth recreation and wellness.

六合社区鈥檚 various offices came together last Friday for a wellness activity that brought us together at one of our Grantee Partner sites, Veggielution. The annual activity is a chance to get together outdoors and feel a part of the processes that we support.听

From weeding the land where fruits and vegetables will be planted, to packaging the locally grown harvest for distribution, we all contributed to a fellow local nonprofit that shares our mission of building health equity.听

Time spent in the office and at the computer, while essential, can sometimes feel abstract and not hands-on. Seeing the outcome of our work in the form of a thriving community farm is refreshing. We are pleased to be a small part of increasing access to fresh fruit and vegetables through trusted partners in health such as Veggielution.听

What is ? The mission of Veggielution is to connect people from diverse backgrounds outdoors through food and farming to build community in East San Jos茅. The farm is a safe, accessible and well-maintained gathering space for the East San Jos茅 community to reconnect to the land and to each other. They offer cooking classes, mentorships, and so much more!听

This year, the World AIDS Day theme, Remember and Commit, is a reminder of the global struggle to end HIV-related stigma, an opportunity to honor those we have lost, and a call to commit to working towards continued medical advances.听The importance of HIV testing cannot be understated 鈥 knowing your HIV status and receiving support is more accessible than ever. Through our HIV/AIDS Services, we are often able to help clients live symptom-free and prevent the spread of HIV. We offer access to a monthly free grocery outlet and can help guide people living with HIV to a variety of needed resources. If you or someone you care about needs support, we are here to help.听

More about why we do what we do: We are building health equity in Silicon Valley – where everyone has the opportunity to live a healthy life by making health-related grants, advocating for policies, and providing services with a focus on food and nutrition, chronic disease prevention and management, and housing support. Because everyone鈥檚 health matters!

For AIDS/HIV Service individuals, we provide a monthly free grocery option where fresh produce and a variety of other healthy options are available. Some of our volunteers (pictured above) have been participating in free grocery days for as many as 20 years!

六合社区 is one of the largest, oldest, and last remaining HIV/AIDS Services organizations south of San Francisco, providing medical and non-medical case management, and nutrition, housing and emergency support services to more than 550 clients each year.

听This year鈥檚 is 鈥World AIDS Day 35: Remember and Commit.鈥 This annual event serves as a reminder of the global struggle to end HIV-related stigma, an opportunity to honor those we have lost, and a rallying cry to commit to working toward a day when HIV is no longer a public health threat.
