ÁùºÏÉçÇø Awards $3.6 Million in Grants to Address ÁùºÏÉçÇø Disparities for Families, Individuals, and Older Adults in Santa Clara and Northern San Benito Counties

San Jose, CA  – In Fiscal Year 2024, ÁùºÏÉçÇø partnered with 22 community agencies by investing $3,500,000 in ÁùºÏÉçÇø Partnership Grants. This Fund supports organizations, programs, and projects for medically-related purposes that align with ÁùºÏÉçÇø’s mission and focus areas of food and nutrition, chronic disease management, housing support, and other approaches to building health equity throughout Santa Clara and Northern San Benito Counties. In addition, we awarded 59 Community Grants totaling $100,000 to support mission-aligned events and projects.

Quarter 4 Grant Awards 

As a part of our total investment, on June 26, 2024, ÁùºÏÉçÇø’s Board of Trustees approved 7 new grants totaling $655,500. Two ÁùºÏÉçÇø Partnership grants aligned with the food and nutrition focus area have been awarded to Portuguese Organization for Social Services and Opportunities and Jovenes de Antaño Del Condado de San Benito. North East Medical Services and YMCA of San Benito County both received funding under Chronic Disease Prevention and Management. In addition, three grants were awarded out of ÁùºÏÉçÇø’s ÁùºÏÉçÇø Equity Fund, to Guadalupe River Park Conservancy, Latina Coalition of Silicon Valley, and Latinas Contra Cancer.

Together, the approved grants aim to improve health in our community by addressing food insecurity and social needs for older adults; strengthening nonprofit organizational capacity; integrating clinical coordination for people with diabetes; and supporting the health and well-being of children and families. 

ÁùºÏÉçÇø Partnership Grant Awards

Jovenes de Antaño Del Condado de San Benito

To enhance access to nutritious meals and social activities for seniors in San Benito County through their Meals on Wheels program and infrastructure support. 

North East Medical Services

To strengthen its infrastructure and capacity to improve diabetes management, care coordination, and health education among prediabetic, diabetic, and uncontrolled diabetic patients at their Lundy Clinic.

Portuguese Organization for Social Services and Opportunities (POSSO)

To strengthen POSSO’s infrastructure by investing in organizational effectiveness activities, including an organizational assessment, financial evaluation, and HR policies and procedures.  

YMCA of San Benito County

To address childhood obesity and promote lifelong health within San Benito County families through the expansion and strengthening of the ÁùºÏÉçÇøy Family Home program.

ÁùºÏÉçÇø Equity Fund Awards

ÁùºÏÉçÇø Equity grants are designed to support projects that fall outside of our Focus Areas, yet support the health and well-being of communities disproportionately impacted by health disparities.

Guadalupe River Park Conservancy

To further develop and strengthen Guadalupe River Park Conservancy’s organization effectiveness by investing in leadership development and organizational infrastructure.

Latina Coalition of Silicon Valley

To enhance LCSV’s capacity and infrastructure, creating a ripple effect that enhances health equity and improves overall well-being for Latinas in the community.

Latinas Contra Cancer

To strengthen its mission by continuing to provide culturally and community-centered service delivery, and support its organizational capacity through professional development.
